'Access to' schemes

AppId is over the quota
I was wondering if anyone knew what universities run these 'Access To' schemes? Ive seen a lot around such as Bristol, Birmingham, Leeds, York but I cant find a list of every university that offers it.
Is it just the one scheme that these universities are part of, i.e. is it a nationwide scheme universities choose to be part of or do the universities set it up themselves and thus can set the different criteria? Cus ive noticed they all say different but similar things about the criteria. (such as fulfilling at least two of the criteria at York but not even one at Bristol?)
Also, what actually is it? Ive seen on Leeds I believe, that they can reduce your offer by 40 points but I dont think any of the others ive seen have specified what they actually do?
Basically, if anyone has applied via the scheme or knows a bit more about it then I do then id be grateful for any advice you give!
